| Patty Ordway-Bennett is from Governeur and that is how she rolls | Ann Taormina Miles is Grannie Annie | Susan ThibodeauYoung Susan is currently pursuing her CAS at SUNY Oswego. She obtained a Master’s Degree in Literacy from SUNY Oswego and a Bachelor of Science in Psychology and Special Education from LeMoyne College. She was a high school Special Education Teacher for the past eight years. She is extremely passionate about creating strong and meaningful transition plans for students with disabilities.

ROLES OF TRANSITION TEAM
· Strengthen linkages with area service providers
· Develop individual career plans with students and their families
· Provide community resource information
· Collaborate with education and community partners, students and families
· Encourage parents to actively participate in transition planning
· Alert students and parents to area, low/no cost health and human
services
· Link students with employment opportunities, job training, and placement services
